  • Insurances effective first of month following hire date

(*) Perdiem status exempt from payroll deductible benefits and are non-qualifying except as indicated:

Medical and Vision Insurance: Community Health Plan of Washington—a managed care plan, or the Uniform Medical Plan---a preferred provider network. Samaritan Hospital outpatient services—co-pay waived except $75 for Emergency Room or $100 Surgery.

Dental Insurance: Delta Dental WA Dental Service with $1,500 maximum per year. Orthodontics at 50% -- capped at $1,000 lifetime.

Life Insurance: Value is annual base salary for death by illness or accidental death, (term insurance with Reliance Standard). Included with full medical/vision/dental package or at $.135/$1,000 of annual base salary. Family Premium: $1.46 per month -- covering spouse for $5,000 and $2,000 on child(ren) to age 26.

Voluntary Accidental Death & Dismemberment Insurance: $.019/$1,000 for self $.039/$1,000 for family coverage with Reliance Standard.

Voluntary Supplemental Life Insurance: Elective coverage for family members with Prudential.

Voluntary Personal Insurance: Group rates with METLIFE or Liberty Mutual Auto, recreation vehicles and home-owner’s/renter’s insurances.

Deferred Compensation Plan: 457 self-directed retirement plan at 90 day’s employment and Guided Portfolio Services.

College Savings Plan: 529 College Savings Plan.

Retirement: Employer contribution to Pension Plan effective at one year’s employment, with a 7 year vesting schedule.

Flex Benefits: Spending accounts for dependent care or non-reimbursed healthcare expenses.

P.T.O. (Paid Time Off): Accruing rates

  • Four sick days
  • Eight holidays
  • Two to four weeks year/vacation—(prorated for part-time)
